Relapse Prevention Counselor

Heritage Court, a 70 be facility is looking to add a Behavioral Health Relapse Prevention Counselor to their team. The Prevention Counselor is responsible for providing individualized support and guidance to residents in recovery. In collaboration with the Behavioral Health team, the Behavioral Health Relapse Prevention Counselor will focus on building relationships with residents and focus on strategies to prevent relapse and promote long-term sobriety.

Essential Functions and Responsibilities:

  • Assessment and Evaluation: Conduct thorough assessments to identify clients' risk factors for relapse, including personal history, triggers, and coping mechanisms.
  • Individual Counseling: Provide one-on-one counseling sessions to develop personalized relapse prevention plans, emphasizing self-awareness, coping skills, and stress management.
  • Group Facilitation: Lead educational and support groups focused on relapse prevention techniques, including recognizing triggers, developing coping strategies, and building a support network.
  • Care Conferences: Lead care conferences with required stakeholders and provide progress, insights on current barriers and updates on compliance.
  • Crisis Intervention: Offer immediate support and intervention in crisis situations, employing de-escalation techniques and ensuring the safety of clients.
  • Collaboration: Work closely with other behavioral health team members, including therapists, medical staff, and social workers, to provide comprehensive care.
  • Education and Training: Educate residents and their families about the recovery process, including the importance of aftercare and ongoing support networks.
  • Documentation: Maintain accurate and up-to-date client records, treatment plans, and progress notes in compliance with facility policies and regulations.
  • Continuing Education: Stay informed about the latest research and best practices in addiction treatment and relapse prevention. Knowledge, Skills, and Abilities:
    • Knowledge of addiction theories, recovery processes and evidence-based practices in relapse prevention.
    • Able to manage and provide redirection of potential behaviors of residents in a safe and effective manner.
    • Ability to create care plans and effectively document changes in condition
    • Possesses superior interpersonal, human relations, and behavior management skills.
    • Proficient in crisis intervention and conflict resolution.
    • Ability to quickly build rapport and trust with residents.
    • Works effectively in a collaborative setting.
    • Possesses excellent oral, written, and presentation communication skills.
    • Able to work well under pressure, problem solve, and meet deadlines.
    • Able to remain flexible and adapt to changing demands.
    • Able to communicate with residents, staff, customers and vendors in a courteous and professional manner.
    • Able to maintain confidentiality and use good judgment.
    • Demonstrates a commitment to professional growth and lifelong learning. Education and Experience:
      • High School Diploma or equivalent required.
      • Associate or bachelor's degree in field of Psychology, Counseling, Social Work or related field preferred.
      • Certified Alcohol & Drug Counselor Level I (CADC-I) preferred.
      • Minimum of 2 years of experience in addiction counseling or related field with a focus on relapse prevention preferred.
      • Experience working with adults with developmental disabilities and/or moderate to severe mental illness preferred.

        Physical Requirements:

        Must be able to move intermittently throughout the workday.

        Must be able to speak and write the English language in an understandable manner.

        Must possess sight/hearing senses or use prosthetics that will enable these senses to function.

        Push/pull/life and carry a minimum of 25 pounds to height of 5 feet for a distance of 25 feet.

        Must be able to sit for intermittent periods of time.

        Must be able to stand and/or walk frequently.

        Must be able to bend at the waist.

        Must be able to repetitively use hands or adaptive devices for data entry.
