Job Description:
A company is looking for a Care Coordinator RN to work remotely
with licensure in Louisiana.
Key Responsibilities:
Provide patient-centered interventions and develop care plans
Monitor member adherence to care plans and collaborate with the
multidisciplinary team
Assist members with scheduling appointments and follow-up care
Required Qualifications:
Licensed RN or compact RN license in Louisiana
2+ years of clinical experience
2+ years of experience with clinical documentation systems
2+ years of experience in assessing members and developing care
plans
Ability to work Monday through Friday, 8am-5pm CST
