Job Description:
A company is looking for a Medical Assistant Patient CCM Care
Coordinator.
Key Responsibilities
Manage and support patients' healthcare needs through phone
conversations
Complete individualized patient care plans and perform care
management services
Document all communications with patients, caregivers, and
providers accurately
Required Qualifications
Certified Medical Assistant, Nationally Certified Pharmacy
Technician, or LPN with compact license
At least 2 years of work experience in a relevant role
Knowledge of healthcare terms, conditions, and basic care
principles
Experience working with Electronic Medical Records (EMR)
software
Intermediate to advanced computer skills
